DESCRIPTION
Mankind discover the existence of the Vampire and Lycan species and they begin a war to annihilate the races. When Selene meets with Michael in the harbor, they are hit by a grenade and Selene passes out. Twelve years later, Selene awakes from a cryogenic sleep in the Antigen laboratory and meets the Vampire David. She learns that she had been the subject of the scientist Dr. Jacob Lane and the Vampire and Lycan species have been practically eradicated from Earth. But Selene is still connected to Michael and has visions that she believes that belongs to Michael's sight. However she has a surprise and finds that she has a powerful daughter named Eve that has been raised in the laboratory. Now Selene and David have to protect Eve against the Lycans that intend to use her to inoculate their species against silver.

Sono trascorsi quindici anni da quando Selene e il suo amore, Michael, un ibrido umano-Lycan, hanno sconfitto l'Anziano Marcus in Underworld Evolution, ma da allora il genere umano ha scoperto l'esistenza sia dei Vampiri che dei Lycan e ha scatenato una guerra senza quartiere per sterminarli. Selene, catturata durante il genocidio, si risveglia dopo oltre dieci anni e si ritrova prigioniera in un laboratorio di Antigen, una potente compagnia biotech impegnata a sviluppare un vaccino contro i virus che hanno creato Vampiri e Lycan.
Selene è distrutta dalla scoperta che Michael è morto, ma il suo dolore presto passa in secondo piano quando viene a sapere che, mentre era in stato criogenico, ha dato alla luce una bambina, Eve (India Eisley). Alla ricerca di un rifugio per sé e per la figlia, Selene scopre che il suo orgoglioso popolo è stato cacciato fin quasi all'estinzione e che i pochi sopravvissuti, nascosti nel sottosuolo, la respingono.
Al fianco di Selene nella sua vendetta contro Antigen, dove forze oscure tramano per sterminare lei e la figlia, si schiera un giovane vampiro, David (Theo James). Ma nei confronti dei nemici di sempre, i Lycan, che ancora una volta stanno riprendendo il predominio, perfino Selene sembra impotente di fronte al più feroce e crudele antagonista della storia di Underworld - un super Lycan geneticamente modificato.
