Durante la Seconda Guerra Mondiale, quattro persone si rifugiano sulle colline intorno a Firenze:
un uomo misterioso dotato di una vasta cultura storica e geografica, reso irriconoscibile dalle ustioni;
la ventenne Hana, infermiera che ha abbandonato il suo distaccamento per accudire lo sconosciuto paziente;
Caravaggio, ladro italo-canadese, e infine l'artificiere Kip, indiano sikh, che diventa l'amante di Hana.
Il paziente misterioso racconta la sua tragica avventura nell'Africa coloniale. Amore impossibile, guerra, esotismo, adulterio, morte romantica, bella fotografia, un po' di tensione "telefonata".
Il romanzo di Michael Ondatjee da cui il film è tratto non è lunghissimo, tre ore sono troppe e soprattutto il film non ha il coraggio di osare il mélo spudorato,anche se i momenti migliori (oltre alle immagini aeree) sono i duetti mélo tra Fiennes e Kristin Scott Thomas.
