Als de kroonprins van Oostenrijk dood wordt aangetroffen, wijst alles erop dat het om een zelfmoord gaat, aldus inspecteur Lestrade. Maar volgens Sherlock Holmes (Robert Downey Jr.) is de prins vermoord, en die moord is slechts een deeltje van een onheilspellende puzzel, bedacht door Professor Moriarty (Jared Harris). Een spoor van aanwijzingen brengt Holmes in een ondergrondse herensociëteit, waar hij en zijn broer Mycroft (Stephen Fry) het glas heffen op Dr. Watson (Jude Law), om zijn laatste avond als vrijgezel te vieren. In die club maakt Holmes kennis met Sim (Noomi Rapace), een waarzegster die veel meer ziet dan ze vertelt. Omdat ze ook meer weet van de moord op de prins, dreigt zij het volgende slachtoffer van de moordenaar te worden.
